The KB-65 is the most common type of intermediate rail fastening for R65 and R50 rails on reinforced concrete sleepers. This fastening uses rigid clamps and is suitable for heavy-duty main lines as well as sidings in lightly loaded industrial enterprises.

The shaped heads of these bolts fit into grooves on the lining flanges. Elastic washers are installed under the nuts of the clamp bolts. A rubber pad is placed between the rail and the metal lining to improve the elastic properties of the fastening. The metal lining sits on an inclined subsurface recess (to ensure rail inclination) that is embedded 15–25 mm into the sleeper body. A rubber pad 6–8 mm thick is placed between the concrete and the lining for electrical and vibration insulation. This pad transfers transverse horizontal forces from the lining through the sleeper pad to the concrete, improving the operating conditions of the embedded bolts and increasing track gauge stability. The lining is secured to the sleeper using embedded bolts. The bolt heads rest on a metal washer embedded in the concrete, which evenly distributes the load when the mounting nuts are tightened. Electrical insulation between the fastening components and the sleepers is provided by a sleeper pad and a PCB sleeve that fits over the shank of the embedded bolt and is recessed into the hole in the metal lining.
